If I am not an in-network provider, I can provide you with a statement to submit to your insurance company for reimbursement. You will need to check with your insurance company prior to your appointment to verify if they will reimburse you for an out of network provider.
Steps to determine your mental health coverage
Step 1
Call your insurance company to verify you have coverage for mental health services on your insurance plan.
Step 2
Provide your insurance company my name "Amber Given" and let them know I will bill a CPT code of 90837.
Step 3
Ask your insurance company what your plan covers. Services may be fully or partially covered. You may have a co-pay for each session or you may need to meet a deductible before your insurance company pays for services.
